• Payments/Fees

  Every effort has been made to make our charges fair and reasonable. Our fee schedule is based on similar charges in our area. The charge for an office visit varies according to the complexity of the problem. Additional charges are made for lab, x-rays and medical supplies. Unless other arrangements have been made prior to your visit, all charges are payable in full at time of service. We accept cash, checks, VISA, Master Card, Discover, or Debit Cards.

• Insurance Information

  We accept most insurance plans. Insurance plans change, so please present your insurance card at each visit to ensure proper payment from your insurance company. IMPORTANT: The insurance organizations listed below typically have several plans they offer. It is the patient's responsibility to verify in-network coverage with a representative of their particular insurance organization prior to receiving medical care. Patients will be responsible for all charges resulting from out-of-network services.

• Specialists

  If you need diagnostic testing including Mamograms, MRI, CT Scans, Bone Density or Ultrasounds we will assist you in making these appointments and obtaining prior authorizations. If your problem requires a specialist, we’ll be happy to recommend one as well as set up your initial appointment. We will also provide the specialist with any necessary medical records from our office. We only assist with your initial appointment. It is the patients responsibility to cancel or reschedule these appointments.
